NEW CLIENTS: Do not type in parent's name as the client. Please type in the child's name. If you have more than one child to register, please only type in ONE child's name per registration card. So if you have 3 kids, you will need to fill 3 of these out one at a time. Please make sure you include the parents' names on the Address 1 line, so we know who to ask for if we need to contact you for any reason. The more phone numbers you can give us the better, so we can reach you incase your stylist is sick, you left your belongings at the salon, etc. If you type in the your child's birthdate, please make sure you type in 4 digits for the year or else it will tell you that the date is too early and will not let you continue with the registration.
